The Case for Vitamin D
Vitamin D benefits are real. This vitamin keeps your bones strong, supports your muscles, and helps your immune system function. But the picture is more complicated than most supplement marketing suggests, and the honest version is more useful.
Most healthy adults are getting enough vitamin D. A meaningful minority, roughly 1 in 4, are low or borderline, and for them, supplementing matters. For everyone else, the evidence for high-dose supplementation is surprisingly weak. Knowing which group you fall into is the whole point of this article.
What Does Vitamin D Actually Do in the Body?
Vitamin D (calciferol) is a fat-soluble vitamin, meaning it dissolves in fat and is stored in your body's tissues. It is biologically inert when you first get it, whether from food, sun, or a capsule. Your body has to activate it in two steps.
First, your liver converts it to a compound called 25-hydroxyvitamin D (also called calcidiol). Then your kidneys convert that into the active form, 1,25-dihydroxyvitamin D, or calcitriol. That is the version your body actually uses.
Its core job is to promote calcium absorption in your gut and keep calcium and phosphate levels in a range that supports normal bone mineralization. Without it, your body cannot absorb enough calcium from food alone, and your bones and muscles suffer. Beyond that, vitamin D plays a role in nerve signaling, muscle movement, immune defense, cell growth, and inflammation control. Many tissues throughout the body carry vitamin D receptors, which is part of why researchers have explored its role in so many different conditions.
How Much Vitamin D Do You Need Per Day?
The Recommended Dietary Allowance (RDA) covers everything combined, including food, sun, and supplements:
Birth to 12 months: 400 IU (10 mcg), listed as an Adequate Intake
Ages 1 to 70: 600 IU (15 mcg)
Ages 71 and older: 800 IU (20 mcg)
Pregnancy and lactation: 600 IU (15 mcg)
The FDA's Daily Value used on supplement labels is 800 IU (20 mcg). These amounts assume minimal sun exposure. The UK recommends a slightly lower 400 IU daily for everyone over 4, so international guidance is not perfectly uniform, but the U.S. numbers are a reasonable target.
Are You Actually Getting Enough?
This is the nuance that most "take vitamin D" articles skip entirely. Dietary intake surveys show that over 92% of men and 97% of women consume less than the estimated average requirement from food and beverages. Those numbers look alarming. But dietary surveys do not capture sun exposure or the vitamin D found naturally in animal foods. When researchers tested blood levels directly, using NHANES data from 2011 to 2014, most Americans came out sufficient.
About 18% of people age 1 and older had levels in the "at risk of inadequacy" range, and roughly 5% were at risk of deficiency. So the real picture is closer to about 1 in 4 being low or borderline, and that gap is concentrated in specific groups, not spread evenly across the population.
The deficiency gap by race is striking. About 17.5% of non-Hispanic Black people were at risk of deficiency, compared to 2.1% of non-Hispanic White people. Skin pigmentation reduces the skin's ability to produce vitamin D from sunlight, which is a major driver of that difference.
Who Actually Benefits from a Vitamin D Supplement?
Two sources answer this well, and they are worth reading together. The NIH Office of Dietary Supplements identifies the groups most likely to be inadequate: breastfed infants (breast milk alone does not provide enough, and the American Academy of Pediatrics recommends 400 IU daily), older adults (the skin becomes less efficient at producing vitamin D with age), people with little sun exposure or who keep their skin covered, people with darker skin tones, and people with fat-malabsorption conditions such as celiac disease, Crohn's disease, cystic fibrosis, or certain liver diseases. People with obesity also tend to have lower circulating vitamin D, because the vitamin gets sequestered in fat tissue.
The Endocrine Society's 2024 clinical practice guideline adds something important. It advises against routinely supplementing beyond the RDA in healthy adults under 75 who do not have a specific clinical reason. But it does identify groups where randomized trial evidence suggests empiric higher intake may help: children and adolescents up to 18 (to prevent rickets and reduce respiratory infections), adults 75 and older (potential to reduce mortality), pregnant people (potential to reduce pre-eclampsia and preterm birth complications), and people with prediabetes (potential to slow progression to type 2 diabetes).
If you are generally healthy, not in any of those groups, and without symptoms, the guideline's position is straightforward. Hit the RDA, mostly from food, sunlight, and fortified products, and avoid supplementing without a clear reason.
What Does Vitamin D Deficiency Actually Do to Your Body?
True deficiency is serious. In children, it causes rickets, which means soft, weak, painful bones that can become deformed. Severe cases can lead to failure to thrive, seizures, and dental problems. In teens and adults, it causes osteomalacia, which is defective bone mineralization that presents as bone pain and muscle weakness.
Milk fortification made rickets rare in the U.S. through most of the 20th century. But cases are rising again globally, particularly in breastfed infants with darker skin and in some immigrant communities where dietary patterns and sun exposure differ. Long-term shortfalls of vitamin D and calcium together also contribute to osteoporosis over time.
Vitamin D also matters for muscle. It is needed for the normal development and growth of muscle fibers, and inadequate levels can cause muscle weakness and pain. That is part of why correcting a true deficiency is worthwhile, particularly for older adults concerned about bone and muscle health. If you are over 50 and working on building or maintaining strength, pairing vitamin D with regular strength training is worth considering. The combination tends to matter more than either alone.
The Honest Scorecard: What Do Clinical Trials Actually Show?
This is where the "vitamin D prevents everything" narrative runs into trouble. Both the Food and Nutrition Board and the Agency for Healthcare Research and Quality concluded the evidence supports vitamin D for bone health, and is inadequate or contradictory for essentially everything else. Here is the trial-level picture.
Bone, falls, and fractures
Vitamin D combined with calcium produces small increases in bone mineral density and reduces fractures in institutionalized older people. In healthy adults living independently, the picture is mixed. The USPSTF found insufficient evidence that supplementation prevents fractures in community-dwelling adults and recommended against low-dose supplementation (400 IU vitamin D plus 1,000 mg calcium or less) for fracture prevention. It also concluded, with moderate certainty, that vitamin D does not reduce falls. The large VITAL trial, which followed 25,871 mostly vitamin-D-sufficient adults taking 2,000 IU per day over 5.3 years, found no reduction in total, hip, or nonvertebral fractures. Some meta-analyses do find a 6% lower fracture risk, and up to 16% for hip fractures, when vitamin D is combined with calcium. The takeaway is that supplements appear to help most when there is a real deficiency, not when levels are already adequate.
Cancer
Supplements do not appear to reduce cancer incidence. VITAL found no difference in breast, prostate, or colorectal cancers. However, several meta-analyses of trials found that vitamin D reduces cancer mortality by about 12 to 13%, particularly with daily dosing rather than large intermittent boluses. The Women's Health Initiative found no incidence or mortality effect at 7 years, but a 7% lower cancer mortality over 22 years of follow-up. The signal, to the extent it exists, is that supplementation probably does not stop you from getting cancer, but may modestly reduce the chance of dying from it. That is a meaningful distinction. Worth noting: very high vitamin D levels have been linked to higher pancreatic cancer risk in some studies, which is another reason to avoid megadosing.
Heart disease
Observational studies link higher vitamin D to roughly 10% lower cardiovascular disease risk per 25 nmol/L increase in blood levels. But trials have not replicated that. VITAL and the New Zealand ViDA trial both found no reduction in heart attack, stroke, or cardiovascular death, even in people who started with low levels. In one finding, doses above 800 IU daily combined with calcium actually raised blood pressure in people with overweight or obesity. The net verdict from trial evidence is no proven cardiovascular benefit from supplementing.
Depression and mood
Low vitamin D is associated with depression in observational data. Trials, however, have not shown a benefit. A meta-analysis of nine randomized trials covering nearly 5,000 people found no improvement in symptoms, and VITAL, which included over 18,000 people, found no effect on depression incidence or mood scores.
Type 2 diabetes
Observational studies link low vitamin D to higher diabetes risk, but trials are mostly negative. The D2d trial, which gave 2,423 adults with prediabetes 4,000 IU per day, did not significantly prevent diabetes overall. A post-hoc analysis suggested a 62% lower incidence in the subset who were deficient at baseline, which is why the Endocrine Society singles out people with prediabetes as a possible-benefit group. The emphasis, though, is on people who are already low.
Multiple sclerosis
The association between low vitamin D and MS risk is one of the more striking findings in this literature, with roughly double the MS risk below 30 nmol/L in some studies. No large trials have tested whether supplements prevent MS, and a 2018 Cochrane review found supplementation did not improve relapse rates or disability in people who already have it.
Weight loss
Higher body weight is associated with lower vitamin D levels, but supplementing does not cause weight or fat loss. If anything, the causation may run the other way: weight loss tends to raise vitamin D levels, not the other way around.
In summary, vitamin D is essential, and correcting a real deficiency clearly matters. There are a couple of modest, still-uncertain signals, specifically around cancer mortality and diabetes risk in people who are already deficient. But for a healthy person with adequate levels, higher doses do not appear to offer additional protection against chronic disease.
How to Get More Vitamin D (Food, Sun, and Supplements)
From food
Very few foods contain meaningful amounts naturally. The best sources, according to the NIH ODS:
Cod liver oil (1 tbsp): 1,360 IU, the single richest source
Farmed trout, cooked (3 oz): 645 IU
Sockeye salmon, cooked (3 oz): 570 IU
UV-exposed white mushrooms (½ cup): 366 IU
Fortified milk (1 cup): 120 IU; fortified soy, almond, or oat milk: 100–144 IU
Fortified cereal (1 serving): about 80 IU
Sardines (2), one large egg, beef liver (3 oz), canned tuna (3 oz): 40–46 IU each
Fatty fish and fish liver oils are the best natural sources by a wide margin. Fortified foods supply most of the vitamin D in the average American diet.
From sunlight
Your skin makes vitamin D when UVB rays hit it. How much you make depends on season, latitude, time of day, cloud cover, air quality, skin tone, age, and whether you are wearing sunscreen, which blocks UVB. UVB does not pass through windows, and UV radiation is the leading preventable cause of skin cancer. Some researchers suggest that 5 to 30 minutes of midday sun on the arms and legs a few times a week may be enough for lighter-skinned people, but this is not a reliable formula for everyone, and it has to be weighed against skin-cancer risk.
From supplements
Two forms exist: D2 (ergocalciferol, derived from yeast) and D3 (cholecalciferol, usually from lanolin in sheep's wool, though lichen-derived vegan versions are available). Both raise blood levels. D3 raises 25(OH)D more effectively and keeps it elevated longer, so it is the preferred form. Because vitamin D is fat-soluble, take it with a meal that contains some fat. If you are also looking to support overall micronutrient intake, note that magnesium is a cofactor in vitamin D activation, and low magnesium intake can blunt how well vitamin D works in the body.
Should You Get Tested for Vitamin D?
Routine testing has become fashionable, but both the USPSTF and the Endocrine Society advise against screening healthy people without a specific clinical reason. No national organization recommends population-wide testing.
Testing is also complicated by assay variability between labs. The same blood sample can read meaningfully different depending on where it is analyzed, sometimes falsely high or falsely low. That uncertainty makes acting on a single number tricky.
Testing makes sense when there is a real clinical reason, such as symptoms that could point to deficiency, a malabsorption condition, osteoporosis, or your provider's clinical judgment. For generally healthy adults without symptoms, routine testing does not add much.
Can You Take Too Much Vitamin D?
Yes, and almost exclusively from supplements. You cannot overdose on sun, because your skin has a feedback mechanism that limits production, and it is nearly impossible to reach toxic amounts from food alone.
Supplement-driven toxicity causes hypercalcemia (too much calcium in the blood), with symptoms including nausea, vomiting, weakness, confusion, bone pain, excessive thirst, and frequent urination. In severe cases it can lead to kidney stones, kidney failure, soft-tissue calcification, arrhythmia, and in rare extreme cases, death. Toxicity is typically associated with blood levels above 375 nmol/L (150 ng/mL).
The Tolerable Upper Intake Level for adults is 4,000 IU (100 mcg) per day from all sources combined. Most standard supplements, in the 1,000 to 2,000 IU range, fall comfortably under that ceiling. High-dose products such as 10,000 IU sit well above the upper limit and should not be taken without medical supervision.
A few drug interactions are worth knowing about. Orlistat (the weight-loss drug) lowers vitamin D absorption. Statins can interfere with vitamin D metabolism. Corticosteroids and prednisone reduce vitamin D levels. Thiazide diuretics combined with vitamin D supplements can raise the risk of hypercalcemia. If you take any of these, ask your provider before adding a supplement.
Taking calcium and vitamin D supplements together also raised kidney stone risk by 17% in the Women's Health Initiative. That is worth knowing if kidney stones are a concern.

For generally healthy adults with adequate levels, the honest conclusion from clinical trials is more modest. High doses do not appear to prevent heart disease, depression, or cancer. There are some tentative signals around cancer mortality and diabetes risk in people who are already deficient, but those are not reasons for everyone to start supplementing heavily.
The practical next step is to check whether you fall into one of the at-risk groups. If you do, a supplement in the 1,000 to 2,000 IU range is a sensible, safe choice. If you do not, prioritize fatty fish, fortified foods, and reasonable sunlight. Talk to your provider before going above 2,000 IU daily, and especially before taking anything near or above 4,000 IU.
